Together, these questions measure satisfaction, usability, whether expectations were met, advocacy, and what to fix next - broad enough to reuse across customer feedback, event evaluation, employee surveys, or product research. Stick to one consistent rating scale (one to five, or zero to ten) and explain what each end of the scale means. Add "not applicable" for anything that won't apply to everyone.

A few more worth adding once you need to go deeper than the core five:
